I can't see this injury being inflicted in the graphic novel nor in the regular novel. I haven't heard it mentioned in spoilers either.

It could be one of the last surprises of the film, seeing as everything else has been leaked now.

Or he scrapes his head on a rock. It could be a lightsaber scar, but it seems more logical that it'd be an actual cut that was so deep it could never heal properly.
